1. Statement about planned participation in the Conference,
2. First and last names, Title, Male/Female
3. Address, telephone and fax numbers, e-mail address,
4. Number and names of accompanying persons,
5. Preliminary title of poster contributions, if you wish to present any,
6. Additional information, if necessary.
